Federal regulations require Child Nutrition Programs Sponsors to offer to all participants’ breakfasts, lunches, suppers, snacks, and milk that meet the meal requirements identified in the individual program regulations.Federal regulations further require Sponsors to make substitutions to the standard meal requirements for participants who are considered disabled and whose disability restricts their diet.The following resources are provided to aid schools in planning, preparing, and serving all Maine students so they can remain safe and healthy.The Federal School Lunch Program is required to provide accommodations for students with developmental and special healthcare needs, including nutrition needs.This includes students who have been identified as having a disability under Section 504 of the Rehabilitation Act of 1973 or under Part B of the Individuals with Disabilities Education Act (IDEA).This also covers students that have been identified as having a disability that restricts their diet by their licensed physician (MD or DO), Physician Assistant (PA), or Advanced Practice Nurse (APN) with prescriptive authority (RXN) but not through Section 504 or IDEA.For more information about what constitutes a disability for the purpose of meal accommodations, please refer to the California Department of Education’s Management Bulletin SNP-02-2017, “Modifications to Accommodate Disabilities in the School Meal Programs.” If a state licensed healthcare professional deems a child’s milk allergy or intolerance to be a disability, then the Medical Statement form must be completed and signed by the licensed physician, physician’s assistant, or nurse practitioner.

The form should be updated by the licensed healthcare professional, as appropriate, to reflect current medical/nutritional needs of the student.

This must identify: The completed form SD1 should be returned to the 504 coordinator, school nurse and/or school foodservice manager.

It will be forwarded to the district’s Registered Dietitian (RD) who will put together a menu/food plan for the student, within 10 days of receiving the SD1 form.

Poway Unified School District is required to make reasonable accommodations for any student with a disability that restricts his/her diet, when supported by a Medical Statement to Request Special Meals and/or Accommodations from a state licensed healthcare professional.

The California Department of Education only permits the following state licensed healthcare professionals to complete and sign the form: licensed physicians, physician assistants, and nurse practitioners.
